The pharmacy’s services are designed to reduce emergency room visits and hospital stays by proactively managing complex medication needs. Care At Home Pharmacy helps clients remain in their own homes longer while providing peace of mind to them and their support networks. Role Description The Staff Pharmacist is a part-time (24 hours or 32 hours per week), on-site role based in Winnipeg, MB. Responsibilities include checking bubble packs, counselling clients and caregivers on medications, performing therapeutic checks alongside our clinical pharmacists, reviewing medication profiles, collaborating with prescribers and other health-care professionals, maintaining accurate documentation and records, and ensuring compliance with all regulatory, safety, and privacy standards. The successful candidate will also function, as time permits, as a clinical resource support Pharmacist with focus on health promotion and additional remote patient care duties as follows: In coordination with patients, caregivers, providers and their assigned clinical homecare pharmacist, select and provide, as appropriate, information and educational hand-outs on medication and health conditions to supplement ongoing clinical pharmacist care and homecare monitoring (e.g diabetes, blood pressure, metabolic tracking sheets for home use) Periodic and proactive screening of therapeutic drug class usage reports and refill utilization history to prompt investigation of potential drug therapy problems among clientele (i.e frequent fills of salbutamol without ICS, long-term PPI use without compelling indication, Tristan overuse and medication overuse headaches etc.) Ongoing development and curation of cost-effective and evidence-based OTC stock for common ambulatory ailments and general health and wellness supplementation. Qualifications Demonstrated clinical pharmacy skills, including medication review, therapeutic optimization, and patient counseling. Strong communication and interpersonal skills, especially with seniors and interdisciplinary professionals Organizational and time-management abilities to manage unanticipated changes to medication bubble packs Attention to detail and commitment to safety for accurate dispensing, documentation, and regulatory compliance. Licensed as a Pharmacist in Manitoba, in good standing with the provincial regulatory body. Proficiency with Kroll and basic office software
