The Community Health Nurse works independently and collaboratively to manage client caseloads, providing assessments, interventions, and health promotion in home and community settings. They facilitate care transitions, coordinate with interprofessional teams, and provide direct clinical care to support client recovery and safety. Candidates must hold current registration as a Registered Nurse with the BCCNM and possess at least one year of recent clinical experience in acute or community settings. A valid BC driver's license and access to a personal vehicle are also required for this position.
