The Community Health Nurse provides direct clinical care, assessments, and care planning for clients in home and clinic settings to manage chronic conditions. They collaborate with interprofessional teams and primary care providers to facilitate transitions and ensure continuity of care. Candidates must hold current registration as a Registered Nurse with the BCCNM and possess at least one year of recent clinical experience with complex geriatric or adult patients. A valid BC driver's license and access to a personal vehicle are required for community-based duties.
