LVN – Client Intake & Community Liaison (Part-Time)

Why You'll Love This Role * Part-time flexible hours * A mix of phone work, home visits, and community outreach * You guide families through a stressful time and help them find the right care * Your nursing background is valued and respected What You'll Do Prospect Calls & Follow-Up * Answer and return calls from families asking about care * Listen to their situation, explain our services, and schedule and conduct intake visits * Follow up with prospective clients who haven't started service yet * Keep call notes and follow-ups up to date in our system Client Intake * Visit new clients and families in their homes to learn about care needs, routines, and preferences * Walk through home safety and help us match the right caregiver * Coordinate care with other health care providers * Document each intake visit * Stay a friendly point of contact for families as care begins if care needs change Community Outreach * Build relationships with local referral sources such as senior living communities, discharge planners, and doctors' offices * Represent Griswold Home Care at occasional community events * Share how our services help families, as a trusted nursing voice What We're Looking For * Active Texas LVN license (required) * Reliable vehicle, valid driver's license, and auto insurance * A warm, confident phone manner, comfortable talking with families and professionals * Organized and good at follow-through * Flexibility for occasional evening and weekend client intakes * Home care, home health, or senior living experience is a plus Sales, marketing, or liaison experience is a plus

Requirements
