Job summary

Using a client-centered approach, the Registered Nurse (RN) provides leadership in the coordination and evaluation of nursing care of patients with multisystem medical conditions awaiting transition to the community setting. The RN works collaboratively with interdisciplinary team members and community agencies to coordinate care and education of patients and their families. Through evidence-based clinical practice and strong leadership in healthcare delivery, the RN sets the standards of excellence in the care of patients at HHS. The RN establishes and implements education plans with and for patients and families and acts as an accessible and knowledgeable resource for patients and their families. Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse Current Basic Cardiac Life Support (BCLS) certification Minimum (1 year) experience in a medical setting within the last 3 years Demonstrated leadership, critical thinking, decision-making, communication, organizational, problem identification and problem-solving skills Demonstrated ability to work independently while collaborating with an interdisciplinary healthcare team Demonstrated ability to manage multiple and shifting demands Venipuncture an asset

Requirements

Candidates must hold a current Certificate of Registration with the College of Nurses of Ontario and Basic Cardiac Life Support (BCLS) certification. A minimum of one year of experience in a medical setting within the last three years is required.
