Facilitate patient transitions back to the community through clinical case management, discharge planning, and resource coordination. Requires a four-year Bachelor’s Degree in Social Work and current registration with the Ontario College of Social Workers and Social Service Workers. Candidates must have at least two years of healthcare experience, specifically working with older adults.
