Role Description The Care Coordinator is a part-time, on-site role based in Ottawa, ON. The role involves coordinating client intake, scheduling, and follow-up appointments, ensuring smooth transitions between services and providers. The Care Coordinator will maintain accurate records, support case management activities, and collaborate with clinicians to facilitate comprehensive care plans. Responsibilities include responding to client inquiries, providing clear information about services, and offering compassionate customer service in person, by phone, and via secure electronic communication. The role also includes liaising with community resources and home care services when needed, while maintaining strict confidentiality and professional boundaries. Qualifications Strong care coordination and case management skills to support client flow, care planning, and continuity of services. Excellent communication and customer service skills for interacting with clients, families, and multidisciplinary team members. Experience with home care or community-based services, including coordinating with external providers and resources. Proficiency with scheduling systems, electronic health records, and basic office software. Knowledge of mental health services and confidentiality standards; prior experience in a clinical or healthcare setting is an asset. Ability to work on-site in Ottawa, ON, manage multiple priorities, and maintain professionalism in a high-performance environment. Relevant post-secondary education in health, social services, psychology, or a related field, or equivalent experience.
