As a result of an upcoming maternity leave at RVH, we are hiring a Temporary Part-Time Registered Nurse our Regional Hemodialysis Program (Renfrew and Pembroke sites). Pay Grid: $40.24 - $57.68 /hour Working as a member of the clinical team and under the direction of the Nephrology Nurse Manager or RVH Coordinators, the incumbent provides and supports patient care delivery in the hemodialysis program. The Registered Nurse provides comprehensive, quality patient/family centered health care services Duties Will Include But Are Not Limited To: The Registered Nurse utilizes the Nursing Process to assess patient care requirements in order to contribute to the formulation of the initial care plan to implement, adjust and evaluate the patient care plan. ASSESSMENT Collecting information about the health status of the patient from: the patient, the family, significant others, members of the health care team and health record by utilizing pre-set forms and procedures systematically. Performing ongoing patient assessment to detect changes in the patient’s condition and/or patient’s needs. Collecting data on the patient’s identified learning needs using the RVH documentation tools and forms. Reporting changes in the patient’s condition to the Physician or Clinical Manager or delegate. Collaborating with other members of the health care team in the assessment process. PLANNING Contributing information and work in collaboration to develop an initial individualized nursing care plan for the patient. Involving the patient and significant others in setting objectives and goals. Modifying the plan of care to address the patient’s needs. Determining the efficient and effective use of resources to carry out the care plan. Contributing to the development of clinical pathways/standard care plans. Identifying priorities for delivery of patient care and re-establish priorities when new or unexpected events occur Qualifications Qualifications: Successful completion of Hospital-based Nephrology Nursing Program and experience in hospital based hemodialysis. Minimum 2 years Medical/Surgical nursing experience and experience with patients with chronic illness. Current Certification in BCLS. Excellent assessment skills and well developed interpersonal skills. CNA (C.Neph.) certification an asset. Experience in primary nursing environment. Formal patient education experience in a structured program. Ability to work independently and to perform duties in the physical demands of work area. Demonstrates effective use of a computer for EHR documentation purposes. Bilingualism an asset.
