The LPN coordinates care delivery for residents, implementing individualized care plans and monitoring physical and psychological conditions. They are also responsible for wound treatment, medication administration, and providing guidance to care staff.

The Social Worker delivers psychosocial services to residents and families in long-term and convalescent care programs. Key duties include managing admissions and discharges, conducting incapability assessments, and coordinating community resources.
