Role Description This is a community based role for a Registered Practical Nurse (RPN) located in Sarnia, ON. The RPN will be responsible for providing care and support to senior clients, including developing and implementing individualized care plans, administering medications, monitoring health conditions, and supporting daily living activities. The role also involves collaborating with families, caregivers, and other healthcare professionals to ensure clients' well-being and addressing any changes in their condition promptly and effectively. The RPN will contribute to creating a safe, compassionate, and respectful environment for all clients. Qualifications * Current RPN license and knowledge of nursing practices and regulations in Ontario * Proficiency in patient care, health assessments, and individualized care planning * Competency in medication administration, wound care, and monitoring of health conditions * Effective communication, collaboration, and documentation skills * Compassionate and patient-focused with a strong commitment to providing high-quality care * Experience in senior care, including dementia, Alzheimer's, or chronic illness management, is an asset * CPR/First Aid certification and compliance with all regulatory health and safety requirements * Proficiency in maintaining confidentiality and adhering to professional standards We are proud to be an equal-opportunity employer.
