On Call Worker

The On Call Worker acts as a first respondent for after-hours home care issues, managing scheduling changes and client communications. They are responsible for documenting all activities in AlayaCare and keeping supervisors informed of any occurrences.

Registered Nurse (RN)/Registered Practical Nurse (RPN)

On-site · Toronto

The Registered Nurse/Registered Practical Nurse provides clinical care assessments, case management, and crisis intervention for home-bound clients. They also act as a clinical consultant for the Home Care team and contribute to quality improvement and infection control planning.

Personal Support Worker (Transitional Care Centre)

On-site · Toronto

Provide direct personal care services including bathing, grooming, and mobility assistance to clients in a 24-hour respite care facility. Additionally, facilitate program activities, document daily care, and monitor client status for any changes.

Clinician (GAIN)

On-site · Toronto

The GAIN Clinician conducts comprehensive geriatric assessments and develops individualized, person-centred care plans for older adults with complex needs. They collaborate with interprofessional teams and community partners to coordinate services, ensure safe transitions, and promote aging in place.

Nurse Practitioner (Family Health Team)

The Nurse Practitioner will provide comprehensive geriatric assessments and direct patient care within an interdisciplinary team. They are responsible for ordering diagnostic tests, prescribing medications, and developing integrative care plans to support frail seniors.

Physiotherapist (Family Health Team)

Provide community-based physiotherapy services including assessment, diagnosis, and education within a multidisciplinary team. Maintain accurate clinical documentation and actively participate in program development and evaluation.

Medical Receptionist (Family Health Team)

The Medical Receptionist provides administrative and clinical support at the front desk, including greeting patients and managing patient flow. They are also responsible for booking appointments, processing OHIP billing, and maintaining patient records within the EMR system.
