Job Description Personal Support Worker Mission The Personal Support Worker (PSW) provides personal care and performs assigned duties within their scope of knowledge and skill, under the direction of their supervisor. The PSW is responsible for delivering care in accordance with regulatory requirements and Extendicare’s standards, policies, and procedures. Key Responsibilities Adhere to the resident/client care plan as determined by the supervisor. Facilitate resident move-ins and transfers, where applicable. Observe, document, and report resident/client care and services using approved Extendicare forms and tools. Assist residents/clients with personal care, hygiene, and activities of daily living (ADLs). Monitor and report changes in residents’/clients’ conditions to the supervisor. Escalate medical emergencies to the supervisor or emergency medical services, as appropriate. Plan and prepare nutritious meals as required. Assist with ambulation, mobility, transfers, and the use of adaptive aids. Demonstrate an understanding of resident/client needs while supporting and maximizing individual abilities. Provide cognitive, social, emotional, and psychological support to residents/clients and their families. Maintain the confidentiality of resident/client information. Respond to resident/client/family concerns and ensure appropriate action is taken within scope of authority, or escalate concerns to the supervisor as required. Help maintain a safe, clean, and organized environment, including resident/client equipment, personal belongings, and nursing equipment. Observe and report any concerns regarding client safety, including environmental hazards, to the Client Service Supervisor; implement interventions as directed and advocate on the client’s behalf to ensure safety is maintained. Collaborate with other agencies and healthcare providers involved in resident/client care, as directed. Participate in in-service training and ongoing professional development opportunities. Work in a position of trust with vulnerable adults and seniors. Uphold and promote Extendicare’s values, philosophy, ethics, integrity, and Code of Conduct. Perform other duties as assigned. Qualifications Graduate of a Personal Support Worker program (or provincial equivalent certificate program with a minimum of 600 hours of theory and practicum in Ontario and Alberta). Experience working with seniors in a community health services setting. Strong written and verbal communication skills. Demonstrated organizational and time management abilities. Excellent interpersonal and customer service skills. Proven knowledge and competency in providing personal care. Current Basic Cardiac Life Support (BCLS) and First Aid certification. Ability to perform the physical requirements of the role, including lifting and transferring clients. Must provide a current Vulnerable Sector Check (including a Criminal Background Check) completed within the past six months, or be willing to obtain one.
