Key Responsibilities
- Provide comprehensive nursing care to patients in community and home care settings.
- Conduct patient assessments and develop, implement, and evaluate individualized care plans.
- Administer medications, treatments, and wound care as prescribed.
- Monitor patient health status and communicate changes to physicians and members of the healthcare team.
- Educate patients, families, and caregivers on health management, treatment plans, and self-care techniques.
- Promote patient independence, safety, and well-being while supporting care in the home environment.
- Maintain accurate and timely clinical documentation in accordance with professional and organizational standards.
- Coordinate care and collaborate with interdisciplinary teams to ensure seamless service delivery.
- Travel between patient homes and community locations as required to provide care.
- Adhere to infection prevention and control practices, workplace safety standards, and nursing best practices.
- Build trusting relationships with patients, families, and community partners to support positive health outcomes.
- Respond effectively to changing patient needs and provide compassionate, patient-centered care in diverse community settings.
Job Qualifications
Our “exemplary standing” with Accreditation Canada held to the highest regard and is a testament to the fact that we uphold the same hiring standards and patient care delivery as the facilities in which we serve.
- Graduate of an accredited Registered Nurse (RN) program.
- Current and unrestricted registration in good standing with the applicable provincial nursing regulatory body.
- Current CPR/BLS certification.
- Minimum one (1) year of recent nursing experience in community health, home care, acute care, primary care, or a related clinical setting preferred.
- Strong clinical assessment, critical thinking, and care planning skills.
- Experience with wound care, chronic disease management, medication administration, and patient education is considered an asset.
- Excellent communication and interpersonal skills, with the ability to build relationships with patients, families, and healthcare partners.
- Ability to work independently and make sound clinical decisions in community-based settings.
- Proficiency with electronic documentation and mobile technology.
- Valid driver's license and access to a reliable vehicle for travel between patient visits.
- Willingness to travel and work in rural and remote communities, including Manitoulin Island.
- Demonstrated commitment to providing compassionate, culturally sensitive, and patient-centered care.
- Flexibility to work a variety of shifts and assignment lengths as required.
If successful, new hires will be required to provide proof of vaccination prior to their start date.
