Job Description: Accountabilities: Responsible for intake and discharge functions of participants. Receive and document all medications upon arrival and ensure safe and secure storage; administer and record the provision of all medications and treatments as performed. Monitor nutritional status of participants and liaise with the caregiver, Program Supervisors, and Community Nurse to resolve concerns. Create or assist in creating Individual Support Plans/Behaviour Support Plans as needed. Answer call signals from participants in the facility and escalate concerns as required. Contribute to maintaining, and improving optimal physical functionality, quality of life and independence for participants. Provide direction, guidance and support to PSW staff. Assist with personal care routines and routines of daily living as required. Maintain safety standards and confidentiality at all times. Administer, in emergency situations, first aid to injured or ill individuals within the scope of competencies; engage emergency services as required. Provide appropriate participant intervention when managing participant responsive behaviours and provide leadership and support to team during crisis or medical emergencies. Liaise with emergency personal and caregivers during participant related medical emergencies. Ensure accurate documentation. Participate in client reassessments. Report any identified needs to team members, Program Supervisor and/or on–call designate, and caregiver. Complete and submit incident reports as required. Support program evaluation and continuous quality improvement activities. Ongoing monitoring of environment for risk and safety issues and report concerns to the Program Supervisor or designate. Provide support and guidance to part-time and/or casual relief staff. Provide training, mentoring and orientation to new staff/volunteers/students. Working Conditions: This is a permanent part-time position based on 23 hours every other weekend. Work will be performed in the facility space. Schedule will include shifts- days and evenings; flexibility is required to meet program requirements. May require some amount of standing, lifting, bending, and carrying of supplies. Requires attention to following safety procedures and regulations. Ability to lift/move up to 20 lbs. May be exposed to disagreeable people, verbal abuse, people with some challenging behaviours and may be exposed to hazardous cleaning materials. Some exposure to bodily fluids should be expected. Normal hours of work: Work schedule will be set by your Supervisor as per program hours of operation including Friday, Saturday and Sunday every other weekend. Work schedule includes a 1/2 hour paid meal break. Education, Qualifications and Certifications: Completed a Community College or University program in Nursing, or related field in health care consisting of four academic years. Current registration with the College of Nurses of Ontario for those with a nursing background. Current First Aid/CPR certification. Geriatric Certificate an asset. Bilingual in French is preferred. Experience and Skilled Knowledge Requirements: Minimum of one (1) year of prior related experience working with cognitively impaired/frail elderly populations. Must be able to lift 20 lbs unsupported Current Gentle Persuasive Approach training. Current Safe Food Handling Certification. Willingness to develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities. Work in compliance with OH&S Act and Regulations and abide by The Salvation Army’s health and safety policies and procedures. Ability to undergo applicable screening and background checks successfully that are satisfactory to The Salvation Army, in its sole discretion (i.e., The Salvation Army Abuse Registry) and provide an original copy of a vulnerable sector check search. NOTE: Alternative combinations of education and experience may be considered. Skills and Capabilities: Use a client-centered approach by consistently demonstrating compassion, respect, and empathy in all interactions. High standards of integrity, judgment, and confidentiality. Attention to details. Strong communication skills as well as strong writing and oral skills in English. Represent the organization in a professional and engaging manner and assist personnel whom the position supports in developing those skills as required. Proficiency in Microsoft Office products. Compensation: The target hiring range for this position is $23.10 to $28.88 with a maximum of $34.65. Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
