Job Summary
Apotex 3 is a mix of Physical support, Transitional Behavioural Support and Mental Health Units. The residents on these units need a specialized environment and a specialized care team to help manage their activities of daily living. This work area is an ideal setting for those wanting to understand ageing and the cognitive or mental health changes or major issues present in this population.
Key Responsibilities
Supports, in consultation with the RN/ RPN, the physical and personal care needs of the client including, but not limited to: hygiene, grooming and dressing, toileting, recording nutrition and hydration intake, provision of preventative skin care Supports the social, emotional, cognitive and cultural well-being of the client under the guidance of the direct care team Uses professional judgment, monitors and evaluates client care needs and care goals regularly according to LTC standards, policies, procedures and legislation Initiates and maintains a relationship with the client, their family and other care providers
Qualifications
Current P.S.W. certificate from a recognized community college or educational institution is required Recent placement and/or working experience with the physically and/or cognitively impaired elderly in a health care setting is required Experience with mental health assessment and intervention and working with responsive behaviors due to various types of dementia, delirium, and other mental health issues is preferred Basic computer skills – Meditech/PointClickCare, Microsoft Word and Excel is an asset Ability to work independently and as a part of the interdisciplinary team, and effectively manage challenging situations Excellent interpersonal, communication and problem-solving skills
