Surgical Care Program - Thoracic Surgery Remote Home Monitoring Program The Thoracic Surgery Remote Home Monitoring Program is an Ontario Government funded program aimed at reducing length of hospital stay and preventing re-admission to the hospital for all post-operative thoracic patients. The Thoracic Remote Home Monitoring Registered Practical Nurse is responsible for overseeing and managing thoracic patients outside of traditional healthcare settings. This role involves leveraging digital health technologies and wearable devices to monitor patients' health status and vital signs remotely. The main objective is to allow for continuous tracking and management of post-acute care and other medical needs. The Registered Practical Nurse will work in both inpatient and ambulatory care settings with an emphasis on implementing and maintaining remote monitoring systems for thoracic patients, collaborating closely with healthcare providers, patients, caregivers, Ontario Health West and technology vendors, ensures data accuracy and security of remote monitoring systems, and recruits and onboard post-operative patients into the Remote Health Monitoring (RHM) program. The Registered Practical Nurse acts as a member of the team to advocate and provide the care for patients and their families.

What you bring: Current Certificate of General Registration with the College of Nurses of Ontario Current BLS-HCP certification Minimum of two (2) years recent surgical care nursing experience Experience in thoracic surgery preferred Experience in minor procedures using aseptic technique preferred Recent experience in patient education related to medication administration and the care of drains, and wounds preferred Demonstrated knowledge/skills in surgical nursing principles which includes excellent assessment and technical skills Knowledge/skill in PCA/Epidural certification, venipuncture and I.V. starts preferred Proven problem solving, critical thinking and decision making ability Demonstrated effective organizational skills involving time management and prioritization Ability to be self-directed and function independently in a busy clinic environment Ability to work effectively as part of an interdisciplinary team to attain common goals Excellent communication (verbal and written) and interpersonal skills Demonstrated computer proficiency with Automated Clinical Systems Demonstrated practice and commitment to the principles of patient and family centered care Demonstrated practice and commitment to patient and staff safety at LHSC Demonstrated practice and commitment to LHSC's vision, purpose and values Demonstrated ability to attend work on a regular basis
