The assessment process forms the basis for all activities performed by Home Care, as well as referrals to other appropriate agencies and long-term placement. The position responsibilities and functions are fulfilled with an underlying philosophy of engaging the client and family members, encouraging client empowerment and self-efficacy. Client empowerment is achieved with self-care models, and utilizing Client Centered Care Planning with client goals and outcomes. Confidentiality and HIPA guidelines are maintained at all times. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement" Human Resources Exemption: No Education * Degree in a health or social science related field. Requirements
