Function Summary

Reporting to the Director, Family Health Services, this role:
  • Works collaboratively with members of the health care team in providing healthcare for family medicine patients
  • Provides specialized care to an identified group of patients who are at risk
  • Participates in committees which address the needs of at risk patients
  • Provides education to learners and staff

Required Qualifications

  • Bachelor's Degree in Nursing
  • Master's degree for graduates after 2011
  • Valid certificate of registration from the College of Nurses of Ontario (CNO) as NP-PHC
  • Membership in Nurse Practitioners' Association of Ontario (NPAO)
  • Primary Health Care Nurse Practitioner Certifcate
  • Professional Liability insurance with Canadian Nurses Protective Society
  • Valid Basic Life Support (BLS) cardiopulmonary resuscitation (CPR) Level C certificate
  • Advanced medical terminology
  • Intermediate computer skills, including in electronic health records
  • Fluent in French and English (Candidates who do not meet this language requirement are eligible to apply for this position and may be considered if there are no other suitable applicants who meet this requirement.)

  • Excellent patient assessment, critical thinking, communication (oral and written), and problem solving skills
  • Practice autonomously according to the NP standards of practice
  • Ability to work in an inter-professional team environment
  • Ability to prioritize, manage time effectively and be flexible in a very active work environment
