The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, and Transitional Care. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 2 sites – the Main site (Church & Bloor St), Harbour Lights and a Specialized Care Centre (Victoria Park & McNicoll).

UNIT CLERK

Part Time – Monday-Friday (8am – 12pm)

POSITION SUMMARY

Reporting to the Patient Care Manager of the Unit, the Unit Clerk performs various administrative and clerical duties to support the patient care unit. Manages day to day staff scheduling as per the collective agreements under the direction of the Patient Care Manager.

QUALIFICATIONS

Grade 12 Diploma plus Community College Business/ Office Administration, Medical Diploma

2 years’ related experience.

Experience in a healthcare environment would be a definite asset.

Knowledge and experience working in a unionized environment and understanding of Collective Agreement.

Proficient in Microsoft Office (Word, Outlook, Excel and PowerPoint).

Process and assist with management of confidential records for clients on admission and until discharge.

Excellent customer service, organization and time management skills.

Ability to work independently and accurately in the presence of frequent interruptions.

Superior communication skills, both written and verbal.

Maintain confidentiality, exercise good judgement and discretion.

Ability to manage frequent changes within a team environment.

Must have the physical strength, flexibility and endurance required to perform the job, as determined by the physical needs assessment of the position conducted by Corporate Occupational Health and Safety.

MAIN RESPONSIBILTIES

Performs various administrative and clerical duties to support the patient care unit.

Maintains the patient record as needed.

Schedules patients’ appointments (external).

Coordinates patient transportation and escorts as required.

Manages day to day staff scheduling as per the collective agreements under the direction of the Patient Care Manager.

Provides effective communication to internal and external clients.

Maintains clerical supplies for the health care team.

Answer high volume of telephone inquiries from patients, families; refer callers to other health care team and/or other appropriate individuals.

Responsible for Patient Care Conference calendar (General/CCC/quarterly, annual or ad hoc).

Participates in continuous quality improvement activities that improve the quality of the patient care environment.

Promotes the maintenance of a safe and healthy workplace.

Provide back-up support to other unit clerks.

Other duties as required.
